2024-25 PEAK Program Information

We are excited you are participating in PEAK during the 2024-25 fiscal year. The program has been modernized for our current times and offer flexibility to go at your own pace. The handbook provides a full description of the program and should be your "go to" resource throughout your work in PEAK.

PEAK 2024-25 Guidebook (pdf): This resource will serve as your one-stop guide. It is a navigatible PDF document that can be downloaded to your device and shared.

Mentor Points Form (pdf): This document is for level 6, 7, or 8 homes to track their mentoring work. This work is necessary to earn or maintain mentor level status in the PEAK program.

KCCI Survey 2024-25: All homes participating in the 2024-25 year, need to have 6 people complete the survey, 2 being direct caregivers (CNA, CMA).

2024-25 Evaluation Document Upload: All homes are able to submit documents for evaluations here.
