2023-24 PEAK Program Information

In a collaborative effort between the PEAK Advisory Group, Nursing Facility Stakeholders, KDADS, and KSU, we are excited to annouce a revised PEAK program. The program now features faster escalating per diems and greater flexibility in moving through program levels. All program resources have a new look and easier to navigate content. The evidence-based, person-centered care expectations remain the same. It is a great time to be involved and work toward quality improvement through person-centered care. PEAK Guidebook: This is an all new resource, that will serve as your one-stop guide. It is a navigatible PDF document that can be downloaded to your device and shared.

Mentor Points Form: This document is for level 6, 7, or 8 homes to track their mentoring work. This work is necessary to earn or maintain mentor level status in the PEAK program.

KCCI Survey 2023-24: All homes participating in the 2023-24 year, need to complete the survey.
